What to expect during your consultation
Your first visit may be as short as 45 minutes or as long as two hours, depending on the complexity of your personal situation and the level of care that you will require. During this first visit, you will meet with your doctor and the rest of our team. Together, we will go over your medical history, answer any questions or concerns you may have, and discuss treatment options with you. We may ask for blood tests or an ultrasound, and men may be asked to provide a sperm sample-but again, we try to keep interventions to a minimum on this day.
What to bring to your first appointment
Please bring copies of all relevant medical files, especially if you have previously been treated for fertility. Alternatively, you can arrange to have these files sent to us in advance. Please also ensure that both you and your partner (if applicable) have valid Health Cards with you.
In 2007, regulations were passed that require you to sign a Health Canada consent form prior to undergoing fertility treatment. This document along with all others required by us will be sent to you prior to your consultation.
In order for your visit to be covered by OHIP, you will need to have a referral from a family physician, gynecologist, or other fertility specialist. Should you be unable or choose not to obtain a referral, please note that a consultation fee of $150/person or $250/couple is payable upon your first visit to the Hannam Fertility Centre.
